I am happy about the frequent contact and coordination of care. I appreciate the calls to plan for discharge and follow up care.
Very easy to contact and access. Good Feedback. Excellent care. Helps to at least have 2 weeks notice of anticipated discharge so we can set tentative follow up appts that will be timely on return. Helps to have at least discharge weight, discharge cal level and discharge activity level, as well as meds and labs within 48 hours of discharge even if full records aren't sent til later. Transition home is key! Thanks for all you do!
